Applying for the Operation Restore Program
IMPORTANT PROGRAM CHANGE: As of July 1, 2009, patient applications will be collected between July 1 and December 31, 2009. These applications will be reviewed by the committee in January /February of 2010. Another cycle of applications will be received January 1 – June 30, 2010; then reviewed in July/August 2010.
