FUE vs. Strip Hair Transplant

There has been a hot debate for many years about FUE vs. Strip hair transpalnt.  Many doctors have their own strong point of view about one versus the other procedure.  Patients complained to me that at times they find contraversial information in online sources about the pros and cons of FUE vs. Strip hair transplantation also.  

It would be interesting to open the subject here in this professional forum and see:

  1. How doctors define the candidacy of their patients for each procedures.  
  2. What patients feel about each of these procedures and what is their underestanding of the best hair restoration option for them.

Non-medical consultants?

In my opinion, the controversial information is mainly coming from non-medical consultants and non-medical clinic owners as a selling factor. Do we really have treatments, which leave no scars or patients who have absolutely NO pain during or after the treatment? Of cause, there are these rare patients with no scaring or patients having not the slightest pain, but is this the majority of our patients? Sometimes I do wonder that there are no solicitors taking those doctors / clinics to court telling these stupid lies. Can we call it misleading the patient? Every proper trained doctor or surgeon does know the pros and cons of a surgery and so he / she should be the only one consenting patients.

Both methods are great and sometimes even a combination of both methods on the same client in the same treatment is ideal.

We should listen to our patient's concerns and decide what is best for them.

The main difference between the FUE hair transplantation and strip surgery is in the way donor-hair is obtained. Generally FUE hair transplant gives greater customer satisfaction as well as recovery. So I will definitely go with FUE hair transplant.
If you decided to go with FUE then back look of your head looks like a Natural and in case of strip surgery  it looks like a Scarring. However FUE surgery is much more expensive than the strip surgery.

Comparison between strip method & FUE
  Follicular Unit Extraction (FUE) method :

When single follicular units are extracted from the donor area and transplanted on the bald area, it is termed as “Follicular Unit Extraction”. Remember, one can only extract the follicles from the permanent hair-bearing zone and not from any other area.

A lot of people think that follicular unit extraction is a blind technique that usually involves a lot of wastage. This is completely true. A punch is mainly used to extract the follicles. Although you would get different kind of punches to extract FUs, you must pick the right size. There is a very thin linear scar in FUT method, whereas in FUE method there are numerous round scars all over the donor site.

Strip Method:

More than 2500-3500 plugs are harvested that equals to 3500-5000 individual hair follicles. One can expect good results in the first attempt around 100-200 sq.cms only, when implanted at a specific density of around 50 FU per sq.cm.

In the Strip method the closure is by telophytic process which leaves minimum scars, there is no wastage in Strip method, hence, our plastic surgeons consider Strip Method better than FUE method.

 FUE stands for Follicular
 FUE stands for Follicular Unit Extraction. It is the most technically advanced, minimally invasive hair transplant procedure available, during which follicles of 3-4 hairs are extracted from your donor area, and implanted into your balding areas. There are no scalpels or scars involved in this procedure what so ever.    On the other hand, the strip method involves slicing out pieces of your scalp, in the donor areas, and then handling the hair follicles and implanting them. The transection rate of this procedure is much higher than FUE, as the hairs are left exposed longer. There is also much higher risk of infection due to the massive scar left on the back of your scalp.

FUE and FUT are both very good options for hair restoration.

There are many discussions and opinions about what is best: FUT or FUE.

My personal opinion is that there is no need for this "competition", both are very safe, well stablished ways to restore hair. In fact, they are only different ways of getting hair from the scalp: FUT through an excision of a strip to produce the follicular units (FUs) and FUE extracting FUs one by one with micro punches of less than 1mm.

Both techniques leaves scar(s) in the donor area: FUT with a long linear scar (that in patients with good elasticity, properly operated with closure under no tension can be very fine and discrete in most cases); and FUE leavinf no linear scar, but hundreds (or thousands) of punctiforme scars that are usually very discrete allowing patients to have a very short haircut.

Patients with a linear strip scar or with FUe scars (some FUE scars can be noticeable, if the punch used has a diameter higher than 1mm or if multiple sessions are done) can camouflage their scar(s) with scalp micropigmentation, if needed, orientated by their surgeon.

Inittially Strip surgery preduced more grafts in one session than FUE, but nowadays, very experienced surgeons can produce megasessions of more than 3,000 grafts with both methods.

In some countries FUE corresponds to maybe more than 80-90% of the procedures. In my country, Brazil, for example, FUT still is the dominant technique.

FUT has been the gold standard for decades and FUE is rising in the last decade and is an actual tendency, even during medical congresses. But we should not say that one is the latest technique, or more advanced, because both can produce good results and the result doensn't depent only of the extraction method, but in reality a lot of other factors are involved as: proper indication, age and family history observed, good anterior hairline design, refined technique during implantation time, among others. 

For all this reasons, in my opinion there is no better technique. Instead of it, there is a better indication for each individual patients. And in some cases of more advanced degrees, secondary cases with limitations in the donor area or poor donor suply, both techniques can be used combined to achive more grafts and offer better results. For this reson, they are not techniques in competition, but instead, complementary to each other.

So, I think a specialist can choose one technique to perform and to defend, but the ideal is to have all opitons available to offer individualized surgical plan for each case.

These are some of my thoughts and I hope it can be useful information.

 It is about proper patient selection. As you all now hair transplantation is booming. Nowadays many physiscians from different specialities (or even without any specilality) are performing it. Most of them have not enough clue about hair but they have hair transplant assistants who are performing 90% of the procedures. Strip method is more difficult because of slivering. I had a conversation with many Turkish hair transplant assistants. All of them are reporting FUE is better (because they do just FUE).
